If you are not registered (or do not hold an exempt status) with the UK Charity Commissioners or are not recognised as being exempt from payment ok UK income tax I must ask that you complete and return the enclosed questionnaire. Please note that if all the documents requested on the form are not provided, we cannot consider your request.
